Question: installing PHP BB

Hi

I was trying to install PHPBB on a different host that supported PHP, but when I was installing it, it asked for:

Database server port:
Leave this blank unless you know the server operates on a non-standard port.

Database name:

Database username:

Database password:

Prefix for tables in database: 

I created a MYSQL database but I don’t know what goas where Confused

Can you help?
